| 内容記述 | Aims：To enhance the decision-making process and clarify the ethical value conflicts encountered by administrative public health nurses who conducted active epidemiological surveys during the novel coronavirus infection (COVID-19) outbreak. Method： Japan Medical Abstracts Society Web was used to search literature from 2020 to 2023 with the keywords “public health nurses”, “active epidemiological investigation”, 13 out of 198 results, and “public health information”,1out of 10 results, A total of 14 documents were targeted. This is a qualitative descriptive study. Based on Fry's (2021) ethical analysis and decision-making model, we described and organized it. Result：14 documents were researched. As a result of the analysis, 10 categories of ethical issues were generated from 43 scenes. For example, “In principle, the policy was for infected people to be hospitalized; however, due to the rapid increase in the number of infected people, hospitals became full, and even if hospitalization was necessary, we had to allow them to receive home treatment.” Considerations：A characteristic of the correlation between ethical value conflicts and public health nurses' emotions is that during intense ethical value conflict, they experience intense emotions that attempt to move them and induce a desire to disengage from the situation. Nonetheless, an enduring desire to discover something persisted. This evolved into the definition of administrative public health nurses' conception of public health nursing practice. The decision-making process of public health nurses confronted with long-term conflicts of ethical values is characterized by uncertainty. However, in accordance with the objective of maintaining public health nursing practice, the decision-making process entails contemplating the present exigencies, which was characterized by a stringent distinction. |

| 内容記述 | 目的：国内の新型コロナウイルス感染症（COVID-19）の積極的疫学調査を行った行政保健師の倫理的課題と倫理的意思決定の促進を促すものの特徴を明らかにすることである。 方法：医学中央雑誌Web版を用い検索語を「保健師」「コロナ」で2020年から2023年で検索し198文献より13件、また「公衆衛生情報」の行政保健師の実践報告の中の10文献より1件の合計14件の文献を対象とした。また、分析は質的記述的研究であり、Fry（2021）の倫理的分析と意思決定までのモデルに基づき分析した。 結果：14の文献の分析の結果、43の倫理的価値の対立の内容から10の倫理的課題のカテゴリが生成された。倫理的課題は、【生死に関わる判断が求められた重責と恐怖、罪悪感、不合理さ、本来の公衆衛生看護活動ができないという思い】等があった。倫理的意思決定の促進を促すものは13項目が生成され、【感染症対策に取り組むための価値の変換】等があった。 考察：倫理的価値の対立と行政保健師の情動と冷静さの関連の特徴では、情動から生じるつらさと冷静さとを内面に共存させ続けながら、公衆衛生看護実践を思考することが、行政保健師の実践の意味となっていた。また、倫理的意思決定の促進を促すものの特徴は、感染症対策に取り組むために価値の変換等があった。 |
